Job Summary:

We are seeking a compassionate and reliable Certified Nurse Aide / Home Health Aide to provide essential personal care and supportive services to clients in their homes. This role focuses on maintaining patient dignity, privacy, and independence while delivering care outlined in the plan of care under the direction of a licensed nurse.

Qualifications:

High School diploma or equivalent preferred
Ability to read, write, and follow directions
Current certification as a Nursing Assistant (CNA) in good standing on the state registry if applicable
Demonstrated competency in required skills and tasks
Compassionate, responsible, and dependable attitude

Essential Responsibilities:

Provide personal care such as bathing, grooming, dressing, and toileting
Assist with mobility, transfers, ambulation, and prescribed exercises
Monitor and record vital signs and intake/output as directed
Perform light household duties essential to patient health and comfort
Observe, report, and document any changes in the patient’s condition or environment
Follow care plans and instructions provided by the RN or other licensed professionals
Maintain accurate and timely clinical notes
Report patient complaints or concerns to the supervising nurse
Attend staff meetings, annual in-services (minimum 12 hours), and orientation sessions
Comply with HIPAA regulations and maintain patient confidentiality

Work Environment & Physical Demands:

Frequent travel to patient homes with varied environments and conditions
Requires ability to lift, bend, and move patients or equipment as needed
Must be able to hear, speak, and see clearly with or without accommodations
May be exposed to moderate noise levels
