Responsibilities:
- Provide direct patient care in accordance with the care plan established by the Registered Nurse or Therapist
- Assist patients with activities of daily living, including bathing, dressing, grooming, and toileting
- Administer medications and treatments as directed by healthcare professionals
- Monitor and record vital signs, such as blood pressure, temperature, and pulse
- Assist with mobility and transfers, using proper body mechanics and equipment
- Prepare and serve meals according to dietary restrictions and individual preferences
- Perform light housekeeping tasks, such as cleaning and laundry
- Maintain accurate and up-to-date medical records

Requirements:
- Valid certification as a Home Health Aide or completion of a recognized training program
- Experience in tube feeding, catheter care, and medical record keeping is preferred
- Ability to lift and transfer patients safely and with compassion
- Excellent communication skills to interact effectively with patients, families, and healthcare professionals
- Compassionate and caring attitude towards individuals with developmental disabilities or chronic illnesses
- Knowledge of caregiving techniques for individuals in assisted living or home care settings
